History

A sixty-seven year old African American female presents with severe hip pain.  The patient states that a year prior she had had back surgery fusing two vertebrae together to alleviate pain from a bulging disc.  The surgery did not relieve the pain and then the hip symptoms began shortly thereafter.  She reports that what started as a slight limp has turned into a waddle when walking.  The hip joint is stiff and she is developing ankle pain.  The pain has gotten so severe that it is not relieved by sitting, standing, or lying down.  She could only lay on her left side and back and she has noticed that the right leg is beginning to turn inward.  Physical examination confirms these findings.
