Case History:

68 yo female patient presented with lower back pain

The patient states that the pain began gradually and worsened over the last few months. Pain increases with movement and worsens throughout the day. Pain is at the lumbosacral region which is also stiff. The patient does not report any numbness or tingling. There is also pain at the right upper gluteal region. There is no lower extremity radiation. This patient does not drink or smoke. She does not have osteoporosis. She is retired. She takes aerobics and has done so at a variety of gyms over the last year. Pain is sometimes worse after working out. Patient was under the care of an orthopedic surgeon who is recommending MRI and possible surgery. She also states that she just discovered that she has a left levorotary scoliosis of 16 degrees which she did not have as a child. She also stated that she had her right kidney removed as a child and there is a large visible scar across her lumbar region and flank.

 

Question: Based on this clinical history, what are some of the differential diagnoses you are considering?
