70 yo female presents with swelling and pain at and above her left knee

Some of the diagnoses you should consider are meniscus or cartilage tear, collateral ligament tear, cruciate ligament tear, suprapatellar bursitis, chondromalacia patella, injury to the quadratus femoris.

 

Physical Exam Findings:

 

Weight: 160, Height: 5’5”       

 

Examination of the left anterior knee revealed severe swelling above the knee cap and mild swelling at the superior border of the tibial plane. There was no swelling evident within the patellofemoral joint. Spasms of the left posterior semimembranosus were also evident. Atrophy, trigger points and fibrosis of the left quadratus femoris resulted in a slight weakness of leg extension. There was no crepitus on knee movement. Valgus and Varus Stress Tests as well as A-P Draw and McMurray’s Test for meniscus injury were negative. There was no pain upon passive manual movement of the patella or with Patella Grind Test for chondromalacia patella.
